The Font of All Knowledge

In 1986, I kept a folder on my desk labelled Font of All Knowledge. Reflex blue and gold. It held the clinical reference material I was building, piece by piece, to answer the questions my clients kept asking — the ones that medicine wasn't answering. Forty years later, that folder has become something I could never have imagined then. But always knew was coming.

There is a question I have been asked, in various forms, for almost four decades.

Sometimes it comes at the end of a consultation, when the person across the table has been given test results that don't tell them anything useful. Sometimes it comes in an email at eleven o'clock at night from someone who has seen three specialists, been prescribed four medications, and still doesn't know why they feel the way they feel. Sometimes it comes from a new client who has already spent years and thousands of pounds trying to find an answer.

The question is always some version of this: Is there anyone who actually knows what's going on?

I have spent my career trying to be that person. The clinician who connects the dots. Who takes the symptom history, the lab data, the lifestyle picture, the medication list, the family history, and finds the pattern that explains everything — not just the loudest complaint. The Font of All Knowledge, as I half-jokingly named that folder in 1986, still in reflex blue and gold.

What I have built in 2026 is the digital version of that folder. Except it is available to anyone, at any hour, without a three-month wait for an appointment.

What the DH Clinical Concierge actually is

The DH Clinical Concierge is an AI clinical education tool, built on the Claude architecture and trained — via an extensive system prompt — on nearly forty years of working with people. It knows the five tests I use in my core programme. It knows the functional reference ranges that conventional medicine doesn't use. It understands the mechanisms: the IDO pathway, the HPA axis, oestrogen metabolism, the methylation cycle, the gut-immune interface.

It also knows the questions to ask before it offers an interpretation. Because that is the part that most AI health tools get wrong.

A symptom in isolation is almost meaningless. The same presenting complaint — fatigue, for example — has a completely different clinical significance depending on whether the person sleeps well or badly, whether their stress history includes sustained cortisol disruption, whether their gut function is compromised, whether their thyroid markers sit in the lower third of the reference range. Context is everything. The Concierge was built to ask for it.

What it is not: a diagnostic tool, a replacement for clinical judgement, or a substitute for working with a qualified practitioner. It cannot order tests. It cannot review your actual lab results. It cannot prescribe, treat, or diagnose.

What it can do is something that has been genuinely unavailable until now — give you an intelligent, evidence-grounded, contextualised first conversation about what might be happening in your body, what testing would show it, and what the next step looks like.

Why this is different from every other health AI

I have used, reviewed, and tested a considerable number of AI health tools. The pattern is consistent. They are broad. They are cautious to the point of uselessness. They tell you things you already know. They hedge every answer until it contains nothing actionable. And they treat every user identically — because they have no clinical framework for doing anything else.

The DH Clinical Concierge was built on a different premise entirely.

The difference is specificity. The Concierge doesn't ask "how are you feeling?" and return a list of possible conditions. It asks which symptoms, in what combination, over what timeframe, with what sleep pattern, what stress history, what gut history. And then it gives you a clinically grounded response — not a Wikipedia summary — about what the pattern suggests and which test would show it clearly.

The suggestion pills on the interface give you a sense of the entry points: "I can't shift belly fat no matter what I do." "I wake up at 3am and can't get back to sleep." "I've been told I have IBS but nothing helps." These are the real questions. The ones people actually have. The Concierge was built to answer them in clinical terms, without jargon, without hedging, without sending you back to Google.

The Noah's Ark principle

There is a strategic dimension to this that I want to be transparent about, because I think it is worth understanding.

Most new products are built in reaction to something — a trend, a competitor, a market gap that has just become visible. This was built differently. The DH Clinical Concierge exists because the infrastructure of a complete, coherent clinical knowledge base was already in place before the tool was assembled. The five-test framework. The 42 clinical tools. The 130+ long-form evidence-based blog posts. The book series. The Resilient Gut System programme. The complete design of a testing-led approach to health that works from first principles rather than symptom suppression.

"Build it completely before the rain starts. The world doesn't know it exists yet. That's an advantage — you're not racing anyone. You're building something that, when complete, will be immediately and obviously different from anything else available."

The Concierge sits on top of that foundation. It is not a chatbot grafted onto a website. It is the access point to a clinical system that has been forty years in development. That is what makes it structurally different — and why I am confident that what has been built here will not be easily replicated by a tech startup that decided health AI was interesting last Tuesday.

What it costs. What it requires.

Nothing. No email address. No subscription. No login.

I made a deliberate decision to keep the public Concierge completely open. The barrier to getting a first intelligent answer about your health should be as close to zero as it is possible to make it. You open the page, you start a conversation, you get a clinically grounded response within seconds.

There is a password-protected version for existing clients — accessible with the programme code — which has access to deeper clinical context. But the public version is the full experience. I want it used. I want people to discover that it is possible to get a genuinely useful answer to a health question from a tool that has real clinical depth behind it.

The thread that runs through forty years

I went back to that 1986 folder recently. The name was right. The colour was right — reflex blue and gold, the same palette that became the Detective Health design system without any conscious decision to make it so. The ambition was right. The only thing that was missing was the technology to make it real at scale.

The technology now exists. And it has been built, in this particular configuration, by someone who has spent nearly four decades working with people understanding exactly what patients need from a system that is supposed to help them.

That is what the DH Clinical Concierge is. Not a chatbot. Not a product. The Font of All Knowledge — finally, actually, available.

The DH Clinical Concierge is live now at detective-health.com/concierge — free to use, no account required. Start with a symptom, a question, or a test you've had that never made complete sense. See what a clinical conversation actually looks like.
