Every investigation starts with the account of the person it is about. These forms are yours. Stephen reads each one himself before your first consultation, so the session is spent on what they show rather than on questions you have already answered.
Complete them in any order, over a few days. Each one sends to Stephen when you press its submit button, and asks for your consent first. There is nothing to save or email. Once a form has sent, a button under it takes you to the next one.
If you use the same phone or computer each time, each item ticks itself off here when it has been received. If you switch devices, tick it yourself with I've sent this.
You don’t have to answer everything. Answer what you can. If you are not sure, a best estimate is fine: just say it is an estimate. Questions about your birth and early childhood (how you were born, breastfeeding, early antibiotics) are worth asking a parent or older relative about. If you would rather not answer something, leave it blank. That is fine: Stephen sees what was left, and it can be talked about when you meet, or not.
On a phone, the forms work held upright. If anything looks cramped, turning the phone sideways gives more room.
