Arabinose — Very High
Clinically significant Candida overgrowth confirmed. Multiple downstream effects likely: Krebs cycle impairment (tartaric acid blocking fumarase), oxalate production, immune dysregulation, and blood sugar instability. Antifungal intervention is the first priority before addressing energy or oxalate markers — they are consequences, not primary problems. Protocol: caprylic acid 2g, oregano oil, berberine 500mg 3x daily. Strict sugar elimination. S. boulardii 10bn CFU. Coordinate with GI-MAP findings.
Succinic Acid (Krebs) — Elevated
Mitochondrial Complex II impairment confirmed. Succinate dehydrogenase requires CoQ10 and riboflavin (B2) — both likely insufficient. Elevated succinate directly correlates with fatigue, exercise intolerance, and poor post-exertional recovery. If tartaric acid is also elevated, yeast is the upstream driver. Protocol: CoQ10 ubiquinol 200-300mg, riboflavin-5-phosphate 50-100mg, full mitochondrial support. This marker responds well to targeted treatment — retest at 3 months.
HPHPA — Elevated
Significant Clostridia overgrowth detected. HPHPA directly inhibits dopamine-beta-hydroxylase — the enzyme converting dopamine to norepinephrine. Dopamine accumulates; norepinephrine is depleted. This is a gut problem expressing as anxiety, mood instability, and poor stress tolerance. Treat the gut, not the mood symptoms. Protocol: high-dose Lactobacillus 100bn CFU+, berberine 500mg, allicin. Vitamin C 2g and copper to support the blocked conversion. Cross-reference VMA — low VMA confirms the norepinephrine depletion.
MMA — Elevated · Serum B12 Normal
Functional B12 deficiency confirmed at the cellular level — serum B12 is misleading here. MMA is the definitive marker. B12 is required for methylmalonyl-CoA mutase; without adequate B12, MMA accumulates even when blood levels appear normal. This finding explains fatigue, neurological symptoms, mood disturbance, and methylation impairment that standard testing missed. Protocol: methylcobalamin 2000-5000mcg sublingual daily. Investigate absorption if history of gut issues. Retest MMA at 3 months.
